The user enters the position and text for each field. The width may be entered also. Otherwise it will be assumed to extend to the edge of the page. For multi-line fields the number of lines is entered.
This information is determined from a printed grid. It is best to print the grid on a spare copy of the form. Otherwise, you can place the grid behind the form to be filled out, and hold them up close to a light bulb. In this case, a good technique is to have an assistant. One person holds up the form and grid and calls out the field information, while the other writes it down. This will make the task go quickly.
