E-Data Collection Survey Engine 4.0 is a flexible tool that you're looking for. This can help you enable you to build survey Web forms in a matter of minutes, complete with a reporting tool that generates attractive, customizable reports, and dynamic filters that allow you to narrow the survey results based on selected criteria to facilitate analysis.

With the e-Data Collection survey system you determine the type of security to apply to each survey and can allow one submission per user if you want, or allow users to take surveys by invitation only. Send bulk e-mail invitations and reminders to take the surveys.
